QUICK GUIDE TO SUSSEX COUNTY SCIENCE FAIR ENTRY FORMS



Selecting the appropriate forms for your science fair project can seem a bit confusing and complicated. Use this guideline to be sure you have the correct and required forms you need. Please submit photocopies of your forms and you should keep the originals.



All projects must submit the following:



  1. Sussex County Science Fair Entry Form

  2. ISEF Form I – Checklist for Adult Sponsor/Safety Assessment

  3. ISEF Form 1B – Approval Form

  4. ISEF Form IA – Student Checklist/Research Plan

The Research Plan should be typed and include:



  1. The question or problem being addressed

  2. The goals or hypothesis clearly stated

  3. A detailed description of the planned research

  4. Procedures to be followed:

    1. Analysis of risks and description of relevant safety procedures to be followed

    2. Procedures planned for data analysis

    3. Bibliography – a minimum of five (5) references from your preliminary review of literature in a standard format.

    4. If Vertebrate Animals will be used; one resource MUST be an animal care resource.





Certain types of research require specific additional forms:



  1. If you will be working with a RESEARCH INSTITUTION or in an INDUSTRIAL SETTING, you will need Regulated Research Institutional/Industrial Setting Form (ISEF Form IC). Make extra copies, as this form will need to be displayed with your science fair project as well as included in your entry packet.

  2. If you are working with HUMAN SUBJECTS, you will need ISEF Form 4.

  3. If you are conducting a survey, the survey MUST be approved by the school’s Institutional Review Board (IRB) and submitted with the packet.

  4. If minors are to be the subjects of the research, PARENTAL CONSCENT forms must be obtained for each individual that are part of your research. These may be submitted with the project but a SAMPLE must be included with the entry packet.

  5. If there is more than minimal risk involved (as determined by the IRB), HUMAN INFORMED CONSENT FORM forms must be completed by all participants (and guardians). A SAMPLE must be included with the entry packet.

  6. If you will be working with VERTEBRATE ANIMALS, you will need ISEF Forms 5A and 5B. You will also need the ISEF Qualified Scientist Form 2. This type of research is very tightly regulated, and MUST BE PRE APPROVED BY THE IRB AND THE SCIENCE FAIR’S SCIENTIFIC REVIEW COMMITTEE (SRC). YOU ARE NOT PERMITTED TO BEGIN ANY VERTEBRATE ANIMAL RESEARCH WITHOUT THIS PRE-APPROVAL!

  7. If you will be working with POTENTIALLY HAZARDOUS BIOLOGICAL AGENTS, you will need ISEF Form 6A. PRE APPROVAL MUST BE OBTAINED BEFORE BEGINNING THIS RESEARCH. YOU ARE NOT PERMITTED TO BEGIN THIS TYPE OF RESEARCH WITHOUT THIS PRE-APPROVAL! A QUALIFIED SCIENTIST form ISEF Form 2 is also required for this type of research.

  8. If your research involves HAZARDOUS CHEMICALS, ACTIVITIES, OR DEVICES, you will need ISEF Form 3. HOWEVER, the research plan must include QUALIFIED ADULT SUPERVISION, risk assessment, and safety procedures. Chemical concentrations and disposal methods must also be included. PREAPPROVAL OF THE IRB AND SCIENCE FAIR’S SRC MUST BE OBTAINED BEFORE BEGINNING THIS TYPE OF RESEARCH!

  9. If you are continuing your project from a previous year, use ISEF Form 7 for CONTINUING PROJECTS.
